Common Used Forms

Title Forms
The following forms are used for titling purposes. Please read the instructions on the forms to ensure accuracy and completeness of the form.
Application for Duplicate Nevada Title VP012 - If you are the last titled owner on the Nevada Title you can request for a duplicate title, as long as there was no Lien Holder on file. Requesting for a duplicate will void any previous issued titles.
(Do not use for out-of-state title.) Bill of Sale VP104 - Used to transfer ownership rights to a new buyer for vehicles 2010 and older. A title or Application for Duplicate Nevada Tile must accompany this form.
Erasure Affidavit VP019 - To correct any errors done on the ownership documents by the person who made the mistake.
One and the Same Affidavit VP185 - To declare a difference in your legal name on the ownership documentation.
Power of Attorney VP136 - To authorize someone as your attorney in-fact to legally sign on your behalf.
Lien Release Affidavit VP186 - For lienholders to release their interest on a vehicle title.
Repossession Affidavit VP020 - For vehicles being sold by the lienholder on file.
(A lien release must accompany the sale documents) Certificate of Inspection/Affidavit of Vehicle Construction VP064 - For vehicles branded as Salvage or Total Lost that need to restore vehicles safety for public road use. Transfer of Title for Estate Without Probate VP024 - For a Next of Kin to transfer ownership rights. Trustee Appointment and Powers Affidavit VP188 - For titles under a Trust. Transfer on Death - Beneficiary's Affidavit for Title VP241 - to appoint a beneficiary to a specific vehicle.
Registration Forms
The following forms are used for titling purposes. Please read the instructions on the forms to ensure accuracy and completeness of the form.
Application for Vehicle Registration VP222 - Registered Owner declaration to maintain required auto insurance.
Duplicate Registration Certificate VP013 - To request a duplicate certificate and/or replacement decal.
Change of Address DMV22 - To update your address on your vehicle registration or id/drivers license.
(Driver Authorization Cards do not qualify for an address change using this form. Changes must be made in person at a local DMV office.) Lost, Stolen or Mutilated License Plate Affidavit VP202 - If your plates were lost or stolen and need replacing or cancelling. Duplicate License Plate Application SP14 - Requesting duplicate active license plates.
License Plate Release SP67 - To release previously issued plate sequence. Emission Control Exemption Application EC008 - When the vehicle is located out-of-state and is presenting an out-of-state smog check or within Nevada in counties that do not require emission testing. Disabled Person License Plate/Placard Application SP27 - To request disabled license plates/placards. Declaration of Responsibility NVL019 - To pay the reinstatement fine of your vehicle registration.
Military Member Forms

Non-Resident, Active Duty Military Governmental Service Tax Exemption Affidavit VP203 - Heroes currently station in Nevada who declare residents to another state on the LES.
Non-Resident Military Spouse Governmental Service Tax Exemption Affidavit VP203s - For heroes spouses who are stationed in Nevada who declare residents to another state on the LES.
Active Duty Military Combat Affidavit VP258 - for combat warriors who were deployed and vehicle registration expired and need to claim exemption to late fees.
